This form is to request Compassionate Leave. Please see the Compassionate Leave Guidelines on our website (under Clergy Resources) for a more detailed description of the leave (time away from the job) and benefits (money) provided. This leave is intended for employees who are experiencing serious illness, or death, in the immediate family.

Compassionate Leave is granted at the discretion of the Bishop, after due consultation.
